Manage my telemetry data

This form is the fallback for when you no longer have the app installed. If you still have MediaForge, use the "Manage my data" buttons in Settings instead — that path proves the request actually comes from your installation.

An admin reviews every request manually — there is no automatic confirmation email. Because this form has no way to verify the install ID actually belongs to you, export requests submitted this way go through extra manual review before anything is sent.
